Anyone that wealthy and disconnected can afford the healthcare if they get sick, which is probably offset by their access to better resources in multiple other ways anyhow, and I assure you that there are people trying to develop ways to live forever. If anyone does, it will probably be expensive and the rich win again, Lews Therin.
They don't need to pay quite as much attention. Human beings are a practically inexhaustible resource and totally interchangeable for enough money. The rest of us are probably more bothered because we'd prefer not to be extras in Fritz Lang's Metropolis.
